Welcome to this survey on martial arts injuries. Whether you have been injured or not, your voice is valuable to our efforts to understand the frequencies and trends of injury within martial arts.

Taking part in the study

In order to participate in this study, you must agree with the following statements. If you do not, you cannot participate. We nevertheless thank you for your time.

This part of the survey uses a table of questions, 

Have you read and understood the information sheet for the study, “Injury surveillance for martial arts – Pilot 1”?
Do you consent voluntarily to be a participant in this study and understand that you can withdraw from the study at any time by contacting Jill Shaw (Jill.Shaw@open.ac.uk) up until one month after your submission, without having to give a reason?
Do you understand that information and quotes you provide will be used (anonymously) for journal articles and public engagement to shine a light on this important issue of sports injuries?
Do you understand that if you share your email address (optional), it will not be shared beyond the lead researcher (Jill Shaw)?
Do you understand that your data will be stored in a password protected OU server, and the (optional) email address removed prior to sharing the data with the research team? The emails will be deleted from the server within 5 years of the study.
